Requesting an account
If you are a licensed independent practitioner and would like access to your patients’ electronic health records at Parkview, please complete the four steps below to set up an account. Through a secure connection, Parkview will receive your signed request, verify eligibility and confirm access. If you have questions or need assistance, please contact our IS Service Center at 260-266-8600.
- Apply for an access request form
To initiate the request for access, please use the fields below to provide valid e-mail addresses for the office manager/physician (first line) and for the individual who is to receive account access (second line).
- Complete the request access form
In return, a request form will be e-mailed to the individual who is to receive access along with the “Protected Health Information Access Agreement”. Required fields are outlined in red. This information allows Parkview Health to verify that individual’s identity for any possible future interaction and determine whether he/she already have a Parkview Health Alliance Partner ID.
The recipient will need to read the “Parkview Health Information Access Agreement” in its entirety. If the recipient agrees with the terms of the agreement, the recipient will need to electronically sign the agreement by entering their name to agree to the terms and conditions. Note: All electronic forms not signed within 30 days will expire and have to be resubmitted for access.
- Request for manager approval
Next, the request will be e-mailed to the to the office manager/physician. The electronic signature of the office manager/physician is an acknowledgement that the recipient is an active member of their staff and requires access to Parkview Health’s electronic health record.
- Allow 14 days to set up the account
This process will take 14 business days. Once your account is set up, the office manager/physician will receive a secure e-mail with the ID and password. The recipient will then be able to access Parkview Health’s electronic health record as an Alliance Partner.
Alliance partner IDs are required to be renewed once a year. Parkview Health reserves the right to terminate access.
