Parkview Health seeks feedback on area health needs

FORT WAYNE, IND. – Sept. 17, 2024 – Parkview Health is launching a Community Health Needs Assessment, a survey that uses public feedback to identify priorities for resources, education and programming. Anyone residing in the Parkview service area is encouraged to take the survey, available online at Parkview.com/CHNASurvey.

As a not-for-profit health system, Parkview uses the Community Health Needs Assessment in its strategic planning process and the development of community health improvement plans. Conducted every three years, the assessment is also shared with community organizations and leaders to assist in their planning and to encourage opportunities for collaboration.

"The Community Health Needs Assessment is a crucial tool for enhancing the health and well-being of our region," said Dr. Sarah GiaQuinta, senior vice president of community health and equity at Parkview Health. "This survey allows us to gain insights into local health outcomes and uncover barriers that individuals face, such as access to nutritious food, transportation, childcare and housing. Understanding the unique needs of each community we serve is essential, and gathering input from those who live in and provide services throughout the region helps ensure that everyone feels heard and represented."

Community participation is vital to the survey, and Parkview values feedback from all residents. To obtain as many responses as possible, the survey is available online, and it will be mailed to random households across northeast Indiana and northwest Ohio. Teams will also be sent out to collect responses in person. As in prior years, the survey is available in English, Spanish, and Burmese. Additionally, this year the survey will also be available in Karen, a dialect of Burmese.

Responses are anonymous and no personal identifying information is required. The survey will take 5 to 10 minutes to complete. Anyone age 18 and older living in one of the counties listed below is eligible to participate.

The assessment will be conducted in the following counties: Indiana: Adams, Allen, DeKalb, Huntington, Kosciusko, LaGrange, Noble, Wabash, Wells and Whitley; Ohio: Williams.
