The Parkview Advantage Clinic (PAC) is a primary care clinic that takes a comprehensive approach to caring for Medicare Advantage patients who have chronic conditions. The PAC treats patients by addressing medical and social needs using a team-based approach. The PAC team includes two physicians, a full-time pharmacist, a licensed clinical behavioral therapist and a nurse navigator. Additionally, the PAC offers increased access to care, including same-day appointments and longer appointment times. Being able to see one of our providers quickly means you can prevent unnecessary emergency room visits and avoid hospitalizations. 

The Parkview Advantage Clinic is best for patients with two or more chronic conditions who have Medicare Advantage plans: United Healthcare (UHC), Humana, Aetna or Anthem.

As a patient at the Parkview Advantage Clinic, you will have access to a nurse navigator, behavioral health resources and pharmacy services.

 

Nurse navigators

Your nurse navigator will:

  • Help answer your questions and connect with your physician for additional information or to address concerns you may have.
  • Connect you and your family with available hospital and community resources.
  • Provide education on disease processes and your treatment plan.
  • Assist with medication management and prescription resources.
  • Follow up after hospital stays and/or emergency room visits.
Behavioral health resources

As part of the behavioral health resources available to patients, a licensed clinical behavioral health therapist will:

  • Answer your mental health questions, address concerns and connect with your provider for additional information.
  • Gain access to traditional behavioral health visits as well as in-office consults.
  • Help with the following:
    • Mood disorder treatment
    • Substance use disorder treatment
    • Grief and loss
    • Sleep disturbance counseling
    • Depression and anxiety
    • Difficult life transitions
  • Coordinate inpatient psychiatric care, if needed.
Pharmacy services

The PAC pharmacist will:

  • Collaborate with you and your provider on medication management.
  • Counsel you on important medication information.
  • Serve as a resource and provide educational information for the care team.
  • Co-manage chronic conditions when directed by your primary care provider.

Frequently asked questions

What is considered a chronic condition?

A chronic condition (also known as chronic disease or chronic illness) is a health condition or disease that is persistent or otherwise long-lasting in its effects or a disease that comes with time.

Who is the PAC for?

The PAC is best for patients with two or more chronic conditions who have Medicare Advantage plans: United Healthcare (UHC), Humana, Aetna or Anthem. The clinic is a helpful option for patients who have been diagnosed with chronic medical conditions such as  diabetesheart diseaseheart failure, chronic kidney diseasearthritis and more.

What if I have a Medicare Advantage plan not listed?

Unfortunately, we would not be able to care for you at the PAC. However, you can be seen by any of our other primary care providers accepting patients.

What if I only have one chronic condition?

As long as you have the correct insurance plan, (Medicare Advantage plans: United Healthcare (UHC), Humana, Aetna or Anthem), we can care for you at the PAC.

Would this clinic replace my current primary care provider?

Yes. Once you are a patient at the PAC, we will provide all your primary care needs.
