Enhanced primary care is a patient-centered model of care focused on preventative services and chronic disease management provided by the PPG – Family Medicine team. As a patient, you can expect:

- Easier accessibility to your care team.
- In-clinic pharmacist to provide medication management, education and support.
- Mental health support from a behavioral health specialist who, in collaboration with a psychiatrist, can help with diagnoses and treatment planning, if needed.
- Healthcare collaboration with a care navigator to coordinate your medical needs within the clinic and across the Parkview health system.
