Diabetes Care Direct

About

Diabetes is an ever-increasing health care issue with approximately 125,000 people living with diabetes in northeast Indiana and northwest Ohio. Increasing access to medical care has been demonstrated to improve health outcomes for individuals with diabetes. To this end, Parkview has assembled a dedicated diabetes care team consisting of a diabetes nurse practitioner, registered nurse, diabetes educator, dietician, and lifestyle coach to provide personalized diabetes care, on-site at area companies that have contracted with Parkview Employer Solutions for specific diabetes related services. The name of this service is Diabetes Care Direct (DCD) and provides care to enrolled employees and individuals covered under an employee’s health plan (e.g. spouses, partners, adult children, etc.) of participating employers on a quarterly basis to align with national standards for diabetes care.

This study is engaging patients receiving DCD services through a mixed methods approach in order to assess the impact of providing diabetes care at the patient’s workplace on the patient’s clinical and psychosocial experience and outcomes. By bringing the services directly to the employees living with diabetes, Parkview aims to improve engagement, reduce lost productivity time, and improve the outcomes of this population with complex needs. DCD providers and contracted employer (non-patient) personnel are also invited to participate in the study to give input on their experience with DCD services.
