Physical exams and consent forms filed by student-athletes in order to participate in athletics in the 2019-20 school year will carry over to the 2020-21 school year, the IHSAA has informed member schools.
The IHSAA Executive Committee cited the "continued stress" on the medical community in the decision to waive the provisions. Any IHSAA Pre-Participation Physical and/or Consent Form completed and signed after April 1, 2019 remain valid. This applies for 2020-21 incoming freshmen, as well.
Students transfering from one high school to another must provide a copy of their 2019-20 forms to be eligible for 2020-21 competition.
Student-athletes who do not have 2019-20 physical and consent forms on file must complete the 2020-21 version for eligibility.
IHSAA Pre-Participation Physicals and Consent Forms completed after April 1, 2020, will still be accepted, if available and desired.
Any questions should be directed to your individual school.
