Employees can pay for certain pre-hire and annual services before their visit with a credit or debit card. We can make a note on your account that services should be paid by the employee.

Our staff will provide authorized services and treat emergent injuries. If no authorization is provided, our staff will treat the patient by following the company account, previously established with your Business Development Consultant (unless there is an account note that an authorization is required).

If no services are listed on your account, we will attempt to call your company for authorization of services to be performed. (This may cause delays for the employee.) If we are unsure what services a patient needs, no authorization is present, and we cannot get ahold of the company, the patient will be referred back to their employer.

We offer all these options. To bill a work compensation carrier, we need the carrier’s name and their address.

If you would like to view your invoice before forwarding it to your work compensation carrier, we do not need to know your work compensation carrier information.

Yes, companies can provide their own forms to be completed during the visit. There is an additional fee to complete external forms. We need to be made aware of this service so we can add a reminder to your account. Some external forms can be saved in your account. Our medical director makes all final decisions on if providers can sign external paperwork.

We ask that a Parkview custom authorization is submitted. To request your custom authorization form, contact our Employer Solutions team,ParkviewOccupationalHealth@parkview.com or 260-373-9017.

Other authorization options: company or TPA designed authorization form, verbal authorization by a company contact at the appointment or verbal authorization by calling the Occupational Health Call Center at 260-373-9300, option 1.

Your completed custom authorization form can be submitted in one of three ways:

• Send to the appointment with your employee (preferred method to avoid delays).
• Email to POHC.CustomerService@parkview.com.
• Fax to 260-458-5836.

Appointments can be made by emailing POHC.CustomerService@parkview.com or calling the Occupational Health Call Center at 260-373-9300, option 1.

If you are an employer attempting to schedule an appointment for an employee, you can complete the Appointment or Results Request Form and then email to POHC.CustomerService@parkview.com. Once the form is submitted, our team will contact you to confirm appointment date, time and location. This form is NOT an authorization for services. Please send a Parkview custom authorization with your employee to their appointment.

Your employees are welcome to call 260-373-9300, option 1 to schedule their own appointment. We suggest they have their custom authorization in hand so they can refer to it and answer questions.

If you use the Agility Employer Portal, this information can be found under the Visits and Scheduling tab on your dashboard.

Negative rapid drug screen results are available shortly after the appointment.

Results for negative non-rapid drug screens generally take 48 hours to process. Results for positive non-rapid and rapid drug screens that are sent for further testing generally take 5-7 business days to process results.

Email encryption protects potentially sensitive information from being read by anyone other than the intended recipient. Information on how to retrieve email encrypted by Office 365.

Agility Employer Portal login 

It is recommended that one company representative be designated as the portal administrator. The administrator will have access to and be responsible for adding new users, removing old users, unlocking accounts, and resetting passwords.

If you are the portal administrator, email ParkviewOccupationalHealth@parkview.com or call 260-373-9017 and we will send password reset emails.

If you are a portal “user,” contact your portal administrator and they can reset your password. Our team can only unlock portal administrators.

To avoid getting locked out, click change password after the second attempt and answer the security questions.
