Transitional Care: Your goal focuses our service

When you enter the transitional Swing Bed Program at Parkview LaGrange Hospital, our first, most important question for you and your doctor will be, “What is the goal for the end of your stay with us?”

The transitional services we provide in the Swing Bed Program are similar to those you would receive during an inpatient hospital stay – nursing care 24/7, rehab therapy, wound care, easy access to lab and x-ray services and more.

Unlike a typical hospital stay, the focus of these services will be to help you regain your strength and mobility while assessing the services or equipment you may need to be able to attain your goal. These needs will vary widely depending on your individual situation. We will work collaboratively with you and your doctor to make sure you have access to everything that is needed and that all is in place when you are ready for discharge.

What is my first step?

Whether you are planning for an orthopedic surgery or other invasive procedure or are nearing the end of a long hospital stay after an acute illness, if you are concerned that you may not be ready to return home right away, talk with your physician about the Swing Bed Program. He or she can call the Case Manager at Parkview LaGrange Hospital at (260) 463-9279 who will gladly work with you, your physician and your family to determine whether the Swing Bed Program is appropriate for your needs.
