Parkview Health Laboratories is a full-service network that includes laboratories at each of Parkview Health’s hospitals as well as at additional patient service centers in the Fort Wayne area.
For the majority of laboratory testing, no appointment is required, but you should have a written order (laboratory requisition or doctor’s script) that includes:
- Patient’s name
- List of tests requested
- Doctor’s name and address
- Doctor’s signature or an authorized signature
- Any special requests
You may go to the laboratory of your choice for testing.

Monday through Friday, 6 a.m. – 7 p.m.
Saturdays, 7 a.m. – Noon
Specimens may be dropped off at any time.
