The Laboratory at Parkview LaGrange Hospital is part of the Parkview Health Laboratory’s full-service network that includes laboratories at each of Parkview Health’s hospitals as well as at additional patient service centers in the Fort Wayne area.
There are two service locations in LaGrange County:

For the majority of laboratory testing, no appointment is required, but you will need to have a written order (laboratory requisition or doctor’s script) that includes:
- Patient’s name
- Diagnosis
- List of tests requested
- Doctor’s name and address
- Doctor’s signature or an authorized signature
- Any special requests
You may come to Parkview LaGrange for your testing or go to any of the Parkview Health Laboratories that is most convenient for you. Hours for outpatient laboratory services at Parkview LaGrange Hospital are Monday through Friday from 7 a.m. through 6 p.m. and Saturdays from 7 a.m. to 11 a.m.
